Physicians at UT Medicine Enter into Accountable Care Agreement with Humana

UT Medicine, the medical practice of the School of Medicine of the University of Texas Health Science Center at San Antonio, and Humana Inc. (NYSE: HUM), one of the country’s leading health and well-being companies, have announced a new Accountable Care agreement.

Under the agreement, Humana members who have local employer-based health insurance will now have in-network benefit access to the 55 primary care physicians affiliated with UT Medicine with increased coordinated and personalized care. Accountable care agreements reflect a shift away from the current episodic health care model to a value-based model that emphasizes quality and patient-care experiences and is designed to control costs.

Members with HumanaOne individual insurance, both on the marketplace health insurance exchange and off the marketplace exchange, may also seek care through this accountable care arrangement.

UT Medicine has six primary care locations serving patients including Medical Arts & Research Center (MARC), 8300 Floyd Curl Dr.; Medical Drive Primary Care, 3939 Medical Dr.; UT Medicine Westover Hills, 11212 Highway 151; UT Medicine Senior Health, 2829 Babcock Road, Suite 525; Family Health Center, 903 West Martin Street, Fourth Floor and General Medicine Clinic, 903 West Martin Street, Third Floor.

“At UT Medicine San Antonio we are committed to building a value-based health care system,” said Carlos Roberto Jaén, M.D., PhD, DABFM, Interim Director of the Primary Care Center and Professor and Chair of Family and Community Medicine at the University of Texas Health Science Center at San Antonio. “Our primary care clinics provide services that fully embody Patient-Centered Medical Home (PCMH) values.

“Our strengthened partnership with Humana will allow UT Medicine to continue on this journey of innovation for the benefit of the patients and communities we serve,” added Dr. Jaén.

“The traditional way that patients have been seen is episodic; the new world is moving us into taking care of patients in a continuum of care encompassing the total health of a patient,” said Drew Kahn, Regional Vice President of Market Development for Humana. “Through a value-based Humana partnership, physicians are able to provide wellness solutions, improved care and a better experience to our members that will lead to improved outcomes like reduction in hospitalization and ER visits.”

The accountable care agreement includes value-based incentives tied to performance improvement and supports successful population health management and a world-class patient experience for Humana members and UT Medicine patients in South Texas. The arrangement offers more personalized care and a broad range of population health tools like predictive analytics, chronic care, disease management, care coordination and wellness programs.

The value-based incentive arrangement emphasizes quality outcomes and leverages standard measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) such as emergency room visits per 1,000, breast cancer screenings, avoidance of antibiotic treatment in adults with acute bronchitis, and diabetes HbA1c testing.

Humana has nearly 30 years of accountable care experience, reflected in results from approximately one million MA members that showed significant improvements in quality, outcomes and costs in 2013. Today, Humana has approximately 1.5 million individual Medicare Advantage and nearly 200,000 commercial members, cared for by approximately 43,000 primary care physicians in more than 900 accountable care relationships across 43 states and Puerto Rico.
