Phytel Adds Large Wisconsin-based Multi-Specialty Group To Growing Client Roster

The health network plans to implement the technology by mid-March.

Through the use of Proactive Patient Outreach, the 65-provider, multi-specialty group will automate the follow up process for its patient population, specifically patients with chronic conditions and those in need of preventative care services. This is accomplished using evidence-based disease management and preventive care protocols to identify patients that are overdue for service. Patients are notified through automated outreach messaging when they are due for recommended care. Proactive Patient Outreach will also enable Holy Family Memorial to track patient response and monitor compliance.
