Transcend Brings Services to Tennessee, Signs Agreement with Healthstar Physicians to Improve Health of Medicare Advantage Members

Transcend brings its population health management services to Tennessee by partnering with Healthstar Physicians to provide value-based care to Knoxville-area Medicare Advantage patients.

Transcend will work with more than 40 Healthstar primary care physicians, consulting with them in their offices to help track, analyze, and act on data about their Medicare Advantage patients’ health. Transcend’s support includes services that will help Healthstar physicians better identify and address opportunities to improve care and health outcomes.

Healthstar was formed in 1995 as a multi-specialty group. Healthstar’s physician network is comprised of more than 75 medical providers representing some 18 medical specialty areas in 14 locations, serving more than 400,000 patients annually across eight contiguous counties.

Healthstar embraced and proactively implemented electronic workflow systems along with quality-based programs the past decade, making them a leader in the healthcare industry. Healthstar physicians and providers are equally focused on disease management and preventive medicine.

The Transcend team will work directly with Healthstar physician office staff to assist with patient data review and analysis, and identify gaps in care. Transcend’s care navigation nurses, for example, will work side-by-side with Healthstar staff to review patient charts and contact patients who are overdue for appointments or would benefit from chronic disease management programs such as diabetes management. Healthstar will also work with Transcend to introduce new efficiencies into their operational procedures in order to more effectively communicate with patients, enabling the delivery of truly coordinated patient care.
