Another Large Multi-Specialty Practice Selects Phytel For Automated Patient Follow-up

Phytel, the leading provider of solutions that automate the patient follow-up process, today announced that Skagit Valley Medical Center, a Washington state, multi-specialty group practice, has selected its Proactive Patient Outreach service.

Through the use of Proactive Patient Outreach, the 80-provider group will automate the follow-up process for its patient population, specifically patients with chronic conditions and those in need of preventive care services. The automated solution utilizes evidence-based guidelines to identify patients and notify these individuals through an outreach messaging system.

For some time, we have been interested in finding more efficient ways to identify patients who need recall exams for conditions such as diabetes or that might qualify for bone density studies, said Larry Thompson, CEO of Skagit Valley Medical Center. We believe Phytel will provide the vehicle necessary to achieve this efficiency and the company is highly regarded by other leading clinics around the country.

Our patients will also benefit by having greater compliance with national standard clinical protocols overseen by their personal physicians here at Skagit Valley Medical Center, added Thompson.

Skagit Valley Medical Center is dedicated to improving the quality of care for their patient population, said Neil Smiley, president and CEO of Phytel. We are confident that Proactive Patient Outreach will quickly prove to be a rewarding solution for both their practice and patients as it has for others.

Phytels Proactive Patient Outreach product integrates with existing clinical information systems, such as practice management, electronic medical records, prescription, lab, and other clinical data. This allows health care organizations to develop and maintain a health and disease management patient registry for an entire patient population. Based on an application service provider (ASP) model, the solution requires no additional equipment and is available through a monthly subscription fee.
