Management in Healthcare conducts peer-review on how company’s post-acute analytics solution helped St. Joseph’s Health ACO achieve a 43% reduction in readmissions and $1.6M cost savings

The article, “Improving patient outcomes while reducing readmissions with data analytics,” highlights how St. Joseph’s Health Accountable Care Organization (Mission Health Coordinated Care), implemented Real Time’s post-acute analytics platform and instituted a post-acute nurse navigator to manage their value-based patients in the post-acute setting. Results showed a 43% reduction in readmissions, 3.2 reduction in average length of stay, and a total cost of care savings of $1.6M as it relates to the Medicare population.
