The Rural Health Transformation Program (RHTP) is a monumental $50 billion federal initiative established through the Working Families Tax Cuts Act (also referred to as the One Big Beautiful Bill Act). Rather than a traditional grant to "keep the lights on," the RHTP is intended to provide a five-year runway, from 2026-2030, for states to build sustainable models focused on preventative health, workforce retention and technological innovation.

Each State Determines Where Funds Go

While the funding is federal, the states design their own transformation plans, resulting in a variety of approaches. For example, while some states are focused on specific clinical providers and services-such as Alabama's mobile cancer screenings or Florida's push for telehealth data-others are using the funds to overhaul financial structures. Notably, 42 states plan to use RHTP funds to consider or implement alternate payment models and value-based care arrangements, like the Achieving Healthcare Efficiency through Accountable Design (AHEAD) value-based care model.

Furthermore, methodologies for distribution differ significantly. For example, while some states require complex proposals, others, like Iowa, are allowing for more opportunity for direct funding allocation to rural hospitals to provide immediate financial relief.

Ultimately, the RHTP empowers each state to develop solutions that are unique to their rural populations. At the same time, it is intended to serve as a bridge to help lessen the impact of reductions in Medicaid funding.
