SOA advised that the following personal identifiable information (“PII”) was exposed: names, dates of birth, Social Security numbers, driver’s license numbers, passport numbers, financial account numbers and/or routing numbers, security codes (CVV), payment card numbers, online account usernames and passwords, PINs or account logins, medical billings/claims information, diagnoses, medical record numbers, Medicare/Medicaid identifications, health information, health insurance information, and patient account numbers.
