Civil Society Role in Global NCD Policy & Fiscal Space: Recommendations Released

Non-communicable diseases cause over 40 million deaths every year, with the greatest burden falling on low and middle-income countries where policy advocacy support remains limited. With these insights in mind, GHAI seeks to address critical gaps in current NCD prevention approaches with its review.

Aaron Emmel delivered remarks on behalf of GHAI at the hearing, which serves as part of the preparatory process for the fourth High-Level Meeting of the United Nations General Assembly on NCDs slated for September 2025. The review outlines three strategic recommendations for effective NCD policy implementation.

The first recommendation emphasizes full civil society engagement in policy planning and implementation processes. GHAI's support of advocacy movements across more than 40 countries demonstrates how civil society organizations provide indispensable evidence, technical assistance, and accountability mechanisms that strengthen government action.

"Our success in reducing the toll of non-communicable diseases will depend on effective policy development and implementation in every country," explained a GHAI spokesperson regarding the organization's advocacy approach.

The second recommendation focuses on expanding fiscal space for NCDs through domestic resource mobilization and strategic health taxation. The review emphasizes that predictable and sustainable health system budgets require strong country ownership supported by civil society advocacy to build political will and public support.

The third recommendation addresses the protection of public health policymaking from health-harming industry interests. The review references GHAI's documented research showing how ultra-processed product manufacturers and other health-harming industries use tobacco industry tactics to prevent and undermine policies that threaten profits.

The comments highlight the need for evidence-based regulations and strong conflict of interest rules to prevent industry interference from slowing NCD prevention progress. GHAI highlights that countries must implement systematic approaches to protect policy development from commercial interests.
