ContrastConnect has released a resource outlining the permanent 2026 CMS rule changes that now allow qualified physicians to supervise contrast imaging procedures remotely through compliant audiovisual technology.

-- ContrastConnect has published a guide on virtual contrast supervision in response to permanent rule changes introduced through the Calendar Year 2026 Medicare Physician Fee Schedule. The updated framework now permits qualified physicians to supervise level 2 diagnostic exams remotely via compliant telecommunications technology — a shift the company says carries significant implications for outpatient imaging centers managing scheduling gaps and coverage shortfalls.

Effective January 2026, the Centers for Medicare & Medicaid Services (CMS) has permanently revised the definition of "immediate availability," which previously required a supervising physician or non-physician practitioner to be physically present for all contrast studies. Under the updated rule, qualified personnel can supervise level 2 diagnostic exams remotely through a real-time audiovisual platform that complies with HIPAA and HITECH regulations. The change follows sustained advocacy from the medical community and is expected to help ease pressure on healthcare institutions dealing with an ongoing radiologist shortage.
"Under the pre-2026 framework, direct supervision meant the physician had to be physically present during procedures. This created a bottleneck that hit smaller practices and rural medical facilities the hardest," a ContrastConnect spokesperson said. "The virtual supervision framework now allows a radiologist or another qualified physician to fulfill that requirement without being on-site for every scan."
The CMS rule change applies to all contrast-enhanced imaging procedures, incident-to services performed by auxiliary staff, and pulmonary and cardiac rehabilitation services. According to ContrastConnect, the changes offer particular advantages to outpatient imaging centers in underserved communities by eliminating travel requirements and scheduling restrictions — allowing a single physician to remotely supervise multiple sites. The company notes that imaging centers performing high volumes of scans without a full-time on-site radiologist can also benefit from the added flexibility.
In addition to improving patient access to care in rural areas, ContrastConnect says that these factors can reduce the logistical burden often experienced by multi-site imaging groups, which must manage scheduling conflicts and coverage gaps across all their locations.
Medical networks looking to adopt the virtual supervision model can partner with ContrastConnect. The platform connects outpatient medical facilities with trained radiologists and provides compliant telecommunications technology, ensuring all health and safety standards are met by its partners.
